Overview
In order to support data checks, external data linkage, and stripping identifiers from free text, the DRC is requesting a number of additional PII tables from the organizations submitting EHR data. These data are not completely captured within the OMOP tables, which are intended specifically for data analysis.
Tables
The data are being requested as a series of tables as some sites may have multiple values per participant for some of these attributes. Each item below is the requested table format. Missing values can be provided as “NULL.” Multiple values for a participant are allowed within each table and are expected in some circumstances. For instance, a participant may have multiple MRNs. All fields are character fields unless noted otherwise.
Name Table
